NEW YORK Mill services provider Tube City IMS LLC has been issued two citations totaling $14,000 after one worker was killed and another injured in an October accident at Nucor Corp.s Norfolk, Neb., facility.
A Tube City employee and an independent contractor were standing in a loader bucket removing a counterweight from a crane when the counterweight fell and struck them, a U.S. Department of Labor spokesman told AMM.
The agency alleges that Tube City exposed the employees to slip and fall hazards and failed to properly secure the crane counterweight. Nucor was not cited.
Each citation carries a proposed penalty of $7,000. Tube City has until the end of the month to pay the penalties, request a conference with the local Occupational Safety and Health Administration office or contest the citations.
A spokesman for Glassport, Pa.-based Tube City declined to comment.
